Behavioural Questionnaire

After receiving the veterinary referral and medical history, I will provide you with our terms and conditions form, along with a detailed behaviour questionnaire for you to complete.

Consultation

We will then arrange a consultation to review all the information, discuss my assessment of the issue, and provide an overview of the treatment plan. This can be done either in person or online.

Behaviour Report

After the consultation, I will prepare a behaviour report summarising our discussion and the treatment plan. A copy will also be sent to your vet for their records.
